Please fill in the following form to register a child for a ‘Quilts 4 Kids’ quilt.

Please note: Child must be nineteen years of age or less and live in Australia.

The only information that will be posted on the ‘Quilts 4 Kids’ website is the child’s first name and last initial, their birthdate, main diagnosis of illness and biography. All other information will be held private and confidential.
